Innovative Integrated Health delivers the Program of All-Inclusive Care for the Elderly (PACE), a community-based alternative to nursing-home placement for older adults with chronic illnesses, disabilities, or needs at the level of nursing-home care. Founded in 2011, the organisation combines medical care and human services for frail and underserved seniors, with the stated aim of helping participants remain safely in their homes while preserving independence and dignity.
The programme covers primary and specialty care, hospital and emergency care, nursing, physical therapy, audiology, optical services, prescriptions and durable medical equipment, alongside social services, counselling, nutrition support, spiritual care, home assistance, transportation and recreational activities. For eligible enrolled participants, the care is covered by Medicare and Medi-Cal with no out-of-pocket costs.
Innovative Integrated Health operates across four California counties: Fresno, Kern, Tulare and Orange. Its care teams are described as culturally diverse and multilingual, working with a multiethnic participant population. The organisation's work sits at the intersection of healthcare, managed care and home and community-based services, and spans clinical, social-service and day-centre functions.
